Intake Forms

The Department of Public Utilities Planning & Engineering Division has developed a centralized system for receiving inquiries and submittals for water, sewer, and/or solid waste projects. Developers or individuals looking to establish, improve, inquire about, or connect to City of Fresno water, sewer, and/or solid waste services should submit the necessary intake forms. Please fill out the appropriate intake form and return it, via email, to the address listed on the form.

The request will be processed in the order received. Please only submit one form per property; this will help avoid delays.
