Hearing Officer Form***

* FPOA Mgmt provides instances where the employee may appeal to a Hearing Officer.  Please refer to the MOU for clarification.

** Please note, use of a Hearing Officer may result in fees associated with the process.

*** Effective July 1, 2010, the fee to obtain a list of neutral hearing officers from the State of California Mediation and Conciliation Service is $50.00. This is in addition to the Hearing Officer fees as specified in FMC 3-283. One half of this fee must be submitted with this appeal form. Please make a cashier’s check in the amount of $25.00 payable to the Personnel Services Department.  Failure to provide a cashier's check when filing the appeal may result in delays in conducting the hearing.
