Self Inspection Program (SIP) Checklist
Verify that there is a minimum 2A-10BC rated portable fire extinguisher which has been serviced and tagged (or replaced) within the past 12 months by a licensed fire extinguisher service company, is wall mounted in plain sight with the top of the extinguisher no higher than 60“ (5 ft.) from the floor, is within 75 ft. of all areas, and the pointer is in the green band on the gauge.
Verify that any extension cords currently in use are in good working order, are for temporary use only (90 days or less), are plugged directly into a wall outlet, and do not pass under rugs, through walls, or through ceilings or ceiling tiles.
Verify that all electrical outlets and switches have cover plates installed, there are no multi-plug adapters in use (remember that single power strips and surge protectors are okay), and there is 30” of clear space in front of each electrical panel.
Verify that (where installed in the building) all fire sprinkler and/or fire alarm components are in good working order, and have not been altered, modified or disconnected.
Verify that if you answered “No” to any of the previous questions, that the conditions noted have been corrected and there are no outstanding fire safety issues or violations.
Verify that you have your occupancy number available. This number is printed on the address side of your SIP postcard. You will need to enter it during your online submission.
Do not forget to submit your form online within 14 days.
