Fresno Area Express (FAX) values the observations and concerns of our passengers and the community. Customer feedback plays an essential role in our ability to adjust or modify transportation services.

How to Complete a Complaint/Compliment Form

The form must be completed in detail. Please include the following information in order for FAX management to conduct a thorough investigation.

  • Customers name, address, and phone numbe
  • Date of incident
  • Time of incident
  • Route number
  • Bus number (if available)
  • Bus direction
  • Location of incident
  • Driver description
  • Detailed information of the complaint/compliment

Formal Complaint form:

  • Print the Complaint Form
  • Complete form in detail
  • Sign and date, return by mail or drop off
